Elder Options to be recognized for work with UF Health
Care One Clinic during 2025
Patient Safety and Quality Week poster session
For several years, Elder Options has partnered with UF Health to address hospital readmission rates of patients with chronic health problems and unmet Health-Related Social Needs, such as housing insecurity, food insecurity, and transportation concerns. Research suggests that patients with unmet health-related social needs face significantly higher readmission rates, particularly if they have one or more social need (ie: housing and food insecurity).
Elder Options works with the patient's primary care team, including doctors, nurses, social workers, psychologists, and pharmacists, to coordinate assistance with social services. Of the patients who participated in the partnership there were no readmissions.
The poster, entitled, "Reducing Admissions/Readmissions by Bridging Clinical Care with Community-Based Care Transition Services through a Partnership between UF Health Care One Clinic and Elder Options", has been accepted for the 2025 Patient Safety and Quality Week poster session. We are thrilled that the efforts of the Care One Clinic and our staff are being recognized in this forum.
