CAREGIVERS - PLEASE TAKE THIS IMPORTANT SURVEY

On behalf of the State of Florida, the Department of Elder Affairs, and the State Health Improvement Plan (SHIP) Priority Area 1, please share this survey with caregivers.

Florida has the second-highest prevalence of Alzheimer’s disease and related dementias (ADRD) with over 580,000 residents with a diagnosis of dementia. ADRD not only affects the individual, but also those who provide care for them and their family. In Florida, we have over 806,000 individuals providing unpaid care for their loved ones. This care is valued at over $20.6 billion.

Florida’s numbers are continuing to grow and are predicted to increase by over 25 percent by 2025. Due to this reality, Florida is doing it all it can to be dementia capable.

Please feel free to share this survey widely. Please access the SHIP Dementia Caregiver Survey here: https://app.smartsheet.com/b/form/5b2b041b2d1244f68dc138f9da85c6f5.
